In some instances, the team will continue to meet at defined intervals to review the data resulting from ongoing monitoring and to evaluate the performance of the process. In other instances, the additional monitoring that is required will be incorporated into the program's ongoing monitoring processes, and the outcomes of the improvements will be reported through pre-existing committees or departmental reports. After the leadership determines that the process changes have been sustained, continued monitoring may be discontinued.

Important questions to be answered in this step include:

  1. Did the process improve as expected?
  2. Did the process improve from the customer's point of view?
  3. Do the data support the improvement?
  4. How could team efforts be improved?
